Abstract
배경/ 목적 : HBV 전파의 주요 감염원이라 할 수 있는 B형 간질환 환자의 가족 및 자녀들의 HBV 보유율은 1980년대 이후 보고된 바 없으며, 특히 감염시 높은 만성화율을 보이는 주산기 감염의 역학에 대한 연구 및 보고 역시 미약한 실정이다. 이에 본 교실에서는 최근 B형 간질환 환자의 자녀들의 HBV 보유율을 조사하고 아울러 이 중 B형 간 질환 여성환자의 0- 19세 자녀들의 HBV 보유율을 조사하여 우리나라 일반 0- 19세 HBV 보유자 중 그들이 차지하는 비율을 추정, 주산기 감염에 의한 보유자의 비율을 알아 보고자 이번 연구를 시행하였다. 대상/ 방법 : 2000년 2월부터 2001년 2월까지 B형 만성 간질환 환자들 및 그 자녀들을 대상으로 문진 및 HBsAg 검사를 시행하였다. 결과 : B형간질환 환자는 총 452명이었으며 이들의 자녀들의 HBV 보유율은 16.8% (184/ 1098)였고, 모계 자녀 감염률은 44.9% (140/ 312)였다. 또한 이들을 20세 이상, 10- 19세 그리고 0-9세로 연령별로 나누어 조사하였을 때 모계 자녀 감염률은 각각 54.7% (2/ 26), 33.3% (28/ 85), 그리고 7.6% (2/ 26)로 자녀의 연령이 적을수록 현저하게 그리고 유의하게 감소하였다. 특히 0- 9세의 모계 자녀 감염률 7.6%는 복지부 발 표자료인 주산기 감염예방 시행 후 HBV 보유율 4.8%와 유의한 차이를 보이지 않았다. 이와 같은 결과로 미루어 판단할 때 일반인구 0-9세의 보유자중 38%, 10- 19세의 보유자중 63.4%가 주산기 감염에 의한 보유자로 추정되었다. 결론 : 현재 사용되는 주산기 감염 예방정책은 그 목표치에 접근하고 있으며 따라서 HBV 보유자 산모와 같은 고위험군의 추적 관리와 함께 백신 무반응자에 대한 대책 및 기존의 백신보다 면역원성이 더 높은 새로운 백신 접종 방법 등에 관한 연구가 필요할 것으로 보인다.
Backgrounds/ Aims: This study aimed to estimate the seroepidemiology of hepatitis B virus (HBV) infection with emphasis on the transmission of HBV infection between mothers and their children. Methods: For 452 patients with HBV associated chronic liver disease, and 1,098 of their offspring, who visited Korea University Kuro Hospital from February, 2000 to February, 2001, HBsAg was tested by radioimmunoassay. Results : Among siblings whose mothers were HBsAg-positive, the overall prevalence rate of HBV infection was 44.9% (140/312) and decreased with decreasing age (54.7% in ≥20 years old age group, 33.3% in 10- 19 years old age group, 7.6% in <10 years old age group). The estimated proportion of perinatal infection out of modes of HBV transmissions in the general population was 38% in <10 years and 63.4% in 10- 19 years. Conclusion: The present hepatitis B vaccination strategies - especially against perinatal infection- of Korea have performed to their utmost. More effective methods for the prevention of HBV transmission are now needed.
