만성 바이러스성 간염환자에서 인터페론 치료로 인한 갑상선 기능이상과 갑상선 자가항체의 발현에 관한 연구

Abstract

목적: 인터페론은 B형이나 C형 간염의 치료제로 사용되어 왔다. 그러나 인터페론의 면역 조절 기능 때문에 치료받은 환자의 상당수에서 갑상선기능 이상을 초래한다고 알려져 있다. 본 연구에서는 만성 B형 및 C형 간염 환자에서 인터페론 치료로 인한 갑상선기능 이상의 빈도 및 갑상선 자가 항체의 출현 빈도를 알아보고 갑상선기능 이상의 발생을 예측할 수 있는 위험 인자의 존재 유무를 규명하고자 하였다.

대상 및 방법: 인터페론 치료를 한 만성 B형 간염 68예와 C형 간염 42예를 대상으로 후향적으로 추적 관찰하였다. 모든 환자들에서 보관된 혈청으로 인터페론 치료 전과 치료 후에 갑상선기능검사(FT3,FT4, TSH)를 하였고 갑상선 자가 항체(anti-thyroglobulin antibody, anti-microsomal antibody)를 측정하였다.

결과: 총 110예의 만성 간염 환자 중 인터페론 치료 후 8명의 환자가 갑상선기능 이상을 나타내었다. 치료 전 갑상선 자가 항체가 양성인 환자는 B형에서 2예, C형에서 1예로 총 3예이었고 치료 중 갑상선 자가 항체는 6예에서 양전되었다. 갑상선기능 이상의 독립적 위험 인자를 찾기 위해 나이, 성별, 인터페론의 투여량과 투여 기간, 간염의 원인, 갑상선 자가 항체 등의 변수를 대상으로 logistic regression analysis를 시행한 결과 치료 중과 치료 전 갑상선 자가 항체 양성이 위험 인자였다.

결론: 만성 바이러스성 간염에서 인터페론 치료 중 갑상선기능 이상이 상당수(7.3%)에서 발생되었다. 아직 비용 대 효과면에서 언제 그리고 얼마 간격으로 갑상선 자가 항체와 갑상선기능검사를 할 것인가가 밝혀져 있지 않지만, 치료 전 이들을 측정해 놓고 치료 중 이들을 추적해서 이상이 생기면 치료 효과와 갑상선기능 이상의 정도를 고려해, 인터페론 투여 중지 여부를 결정해야 하리라고 생각된다.

Background/Aims: Interferons have been employed in the treatment of patients with hepatitis B and C. However, immunomodulatory properties of interferon are known to induce thyroid dysfunction. In the present study, we examined the prevalence of thyroid dysfunction and the development of thyroid autoantibodies following administration of interferon alpha (IFN-alpha), and investigated predictive factors that induced thyroid dysfunction.

Methods: Sixty-eight patients with chronic hepatitis B and 42 patients with chronic hepatitis C were investigated retrospectively. All of them were treated with IFN alpha. Before and after the treatment, FT3, FT4, TSH, antithyroglobulin antibody (Tg-Ab) and antimicrosomal antibody (Ms-Ab) were measured.

Results: During treatment, thyroid dysfunction developed in 8 (7.3%) of the 110 patients. At the start of IFN-alphatherapy, 3 cases had detectable thyroid autoantibodies. During IFN-alphatherapy, 6 additional cases became to have thyroid autoantibodies. Logistic regression analysis was performed for the variables such as age, gender, IFN dose and duration, HBV vs HCV, thyroid autoantibodies. It revealed that positivity of thyroid autoantibodies during and before therapy were statistically significant and independent risk factors for the development of thyroid diseases.

Conclusions: IFN-alphatherapy in chronic viral hepatitis can induce thyroid dysfunction. All patients undergoing IFN-alphatherapy should be screened for thyroid autoantibodies and TFT before therapy and monitored during therapy. The occurrence of thyroid dysfunction requires a decisionwhether to maintain or interrupt IFN-alphatherapy, taking into account the response to therapy and the severity of the thyroid disease.
