Abstract
목적 : 만성 B형 간염의 인터페론 치료후 장기간의 관찰결과는 보고자에 따라 차이가 있으며 서양에서는 반응군의 90%가 e항원의 음전을 지속적으로 유지하며 10% 내지 67%의 환자에서 B형 간염 표면항원의 음전이 일어난다고 보고하고 있다. 국내에서는 만성 B형 간염환자의 인터페론 치료 후 단기간의 성적은 보고된 바 있으나 장기간 추 적관찰 결과가 미흡하며 서구에서의 연구결과를 그대로 받아들이기 어렵다. 본 연구의 목적은 우리나라 만성 B형 간염환자에서 인터페론 치료 후 장기간의 관찰을 통하여 반응군이 비반응군에 비하여 지속적으로 바이러스 증식 표지자와 생화학적 검사소견의 정상화를 유지하는지 비교하고 B형 간염 표면항원의 소실률, 간질환으로 인한 합병증, 간세포암의 발생, 간질환으로 인한 사망 등이 양군 간에 차이가 있는지 알아보고자 하였다. 대상 및 방법 : 만성 B형 간염환자중 인터페론 치료를 받은 68예의 환자를 대상으로 반응군과 비반응군간의 바이러스 증식 표지자(HBeAg, HBV DNA), 표면 항원(HBsAg), 생화학적 검사소견을 추적 검사하였다. 원발성 간암에 대한 선별검사로 복부초음파 검사를 시행하였고 위식도 정맥류 발생을 확인하기 위하여 위내시경검사를 시행하였다. 추적 검사관찰이 종료되는 시점에 간질환으로 인한 합병증, 사망률의 차이를 비교하였다. 결과: 평균 추적관찰기간은 58개월이었다(18개월- 129개월). 반응군은 비반응군에 비하여 지속적으로 e항원의 음전(92% vs 32%, p< 0.05), 생화학적 검사소견의 정상화(86% vs 45%, p< 0.05)를 유지하였다. B형 간염표면항원의 소실은 각각 반응군과 비반응군에서 1예 있었으나 양군의 차이는 없었다(5% vs 4%). 간질환으로 인한 복수, 황달, 위식도 정맥류 출혈, 간성뇌증 등의 합병증 발생은 1예도 없었다. 위식도 정맥류 발생이 9예 있었는 바 2예를 제외하고 모두 비반응군이었다. 간질환으로 인한 사망은 양군에서 각각 1예 있었으며 양군의 차이는 없었다(6% vs 3%). 간세포암의 발생은 반응군 1예, 비반응군 2예 있었으며 양군의 차이는 없었다(6% vs 7%). 비반응군 중 치료종료 1년 이후에 뒤늦게 e 항원의 음전이 일어난 예는 40예중 12예였다. 비반응군 중 e 항원의 음전이 일어난 군과 음전이 일어나지 않은 군간에 치료전 생화학적 검사소 견, 간조직 생검소견 등은 차이가 없었다. 결론 : 만성 B형 간염에서 인터페론 치료는 반응군에서 비반응군보다 지속적으로 e항원의 음전을 유지하며 생화학적 검사치의 정상화를 유지한다. 간질환 합병증, 간질환으로 인한 사망률 등은 양군 간에 차이가 없었다. 따라서 인터페론 치료 반응 여부에 따른 만성 B형 간염에서의 합병증 발생, 원발성 간암의 발생, 사망률 등의 차이를 알기 위해서는 보다 장기적인 추적 관찰이 필요하리라 생각된다.
Background/Aims : Several randomized controlled studies have shown that responders who had treated with interferon alpha for chronic hepatitis B had better rate of sustained loss of HBeAg and HBV DNA than non- responders. These studies also showed that non- responders had higher rates of liver related complication and mortality. But there is very little data on how well sustained responders are and whether the responders eventually lose HBsAg in Korea. The aims of this study were to better define the long term remission of chronic hepatitis B induced by interferon alfa therapy and compare the clinical outcome among the interferon responders and non- responders in Korea. Methods : Sixty- eight patients with chronic hepatitis B who were treated with interferon alfa between 1987 and 1998 were followed up for serologic status (HBsAg, HBeAg, HBV DNA), biochemical tests and liver related complication or mortality. Results : Among 68 patients with chronic hepatitis B who were treated with interferon alfa, 28 (41%) responded to treatment with loss of HBeAg within 1 year of starting treatment. Up to 129 months (mean 58 months) after therapy, responders had higher rate of cumulative clearance of HBeAg at five years than non- responders (100% vs 35.1%, p<0.05). Responders had maintained the normal serum ALT than nonresponders at five years (94% vs 55.6%, p<0.05). Loss of HBsAg was not different between responders and non- responders (5% vs 4%, NS). The rates of liver related complication and mortality did not differ between both groups. Delayed clearance of HBeAg occured in twelve out of forty non- responders (30%). There were no differences in age, baseline ALT, histologic finding of liver biopsy, HBV DNA at the end of first year after study with IFN therapy between the non- responders with and without delayed clearance of HBeAg. Conclusion : Remission in chronic hepatitis B induced by alfa interferon maintained in long duration. But clinical outcomes such as liver related complication, mortality and the elimination of HBV infection have no differences between responders and non- responders. Further studies are needed for the role of interferon therapy in long- term clinical outcome for chronic hepatitis B.
