식도의 분절성 저압성 연동운동과 식도 중간부 게실

Abstract

목적: 식도의 분절성 저압성 연동운동과 식도 중간부 게실과의 상관 관계를 밝히기 위하여 본 연구를 시행하였다.

대상 및 방법: 상부 소화관 내시경 또는 식도 조영술상 식도 중간부 게실이 증명된 환자군 15예와, 식도 관련 증상이 없으면서 연령 및 성별이 환자군과 대응하는 대조군 15예를 대상으로 통상적 식도 내압검사와 mapping 내압검사를 시행하였다.

결과: 통상적 내압검사 결과 비전달 연동파의 빈도는 대조군에 비해 환자군에서 다소 증가된 경향을 보였으나(p=0.0553), 다른 제반 계측치들은 양군 사이에 유의한 차이가 없었다. 통상적 내압검사 진단 기준에 따라 평가하였을 때 대조군은 전예에서 정상 소견을 보였으며, 환자군에서는 5예(33%)에서 비정상 소견을 보였는데, 비특이성 식도 운동장애 4예, 호두까기식도증 1예였다. 비특이성 식도 운동장애 4예 중 1예는 저압성 연동파, 다른 1예는 비전달 연동파의 빈도 증가, 나머지 2예는 이 두 가지 소견을 다 나타내었다. Mapping 내압검사 결과 분절성 저압성 연동운동은 대조군에서는 관찰되지 않았으나, 환자군에서는 15예 중 6예(40%)에서 발견되어 대조군에 비하여 유의하게 많이 관찰되었다(p=0.0225).

결론: 식도 중간부 게실이 있는 환자에서분절성 저압성 연동운동이 드물지 않게 관찰된다. 따라서 식도 내압검사 중 분절성 저압성 연동운동이 관찰될 경우 식도 중간부 게실이 같이 있을 가능성을 생각해 보아야 할 것으로 생각되며, 또 통상적 식도 내압검사에서 정상 소견을 보이는 경우에도 식도의 분절성 운동장애가 의심되는 경우나 게실의 치료를 고려하는 경우에는 통상적 식도 내압검사와 아울러 mapping 내압검사를 같이 시행하는 것이 좋을 것으로 생각된다.

Background/Aims: Esophageal segmental dysmotility has been extensively studied, while reports regarding segmental hypotonic peristalsis are rare. This study was carried out to elucidate the possible relationship between segmental hypotonic peristalsis and midesophageal diverticulum.

Methods: Fifteen patients with midesophageal diverticulum and 15 age- and sex-matched controls were evaluated with conventional and mapping esophageal manometry.

Results: Conventional manometric parameters were similer in the patients and controls. The conventional manometry identified motility disorders in 5 patients (33%) (4 nonspecific esophageal motility disorders and 1 nutcracker esophagus). The mapping manometry showed that segmental hypotonic peristalsis was noted in 6 patients (40%), but it was not found among the controls. Five patients (33%) showed no abnormality from either manometric method.

Conclusions: This observation suggests that segmental hypotonic peristalsis is frequently found in patients with midesophageal diverticulum, although its clinical significance and role in the pathogenesis of diverticulum remain to be investigated. Mapping manometry may be a useful tool in the cases with suspected segmental esophageal dysmotility including diverticulum, even though the conventional manometry shows normal finding.
