- Abstract
- 목적: C형 간염 바이러스(HCV) 감염의 위험성이 적은 자원 공혈자나 건강한 성인에서의 anti-HCV 발현율은 0.3~1.5% 정도로 알려져 있다. 그러나 이들에서 anti-HCV의 발현이 곧 실제 HCV 감염을 의미하는 것은 아니다. 따라서 4-RIBA나 RT-PCR을 이용한 HCV-RNA의 검출이 확인 검사로서 시행되고 있다. 저자 등은 우리나라 성인에서 anti-HCV(2세대 EIA)의 양성 빈도를 확인하고 항체 반응의 특성과 viremia 상태를 알아보고자 한다.
방법: 고대 의료원 건강 진단센터를 내원한 5707명의 성인을 대상으로 sALT, HBsAg와 2세대 anti-HCV(UBI)를 검사하였다. 이들 중 sALT치가 정상이고, anti-HCV가 양성인 예에서 설문조사를 시행하고 이들의 혈청에서 anti-HCV를 추적 검사하고 4-RIBA와 RT-PCR을 이용한 HCV-RNA 검출을 시행하였다.
결과: 우리나라 성인에서 anti-HCV 발현율은 1.77%(101/5707)이었다. 혈청 ALT치가 정상인 예에서 anti-HCV 발현율은 1.45%(71/4893)이었다. Anti-HCV 양성예에서 수혈력의 빈도가 음성예에 비해 유의하게 높았다. Anti-HCV 양성인 28예 중 4-RIBA는 10예에서 양성, 8예에서 indeterminate, 10예에서 음성이었다. HCV-RNA는 28예 중에서 11예에서 검출되었다. 4-RIBA 양성이거나 anti-HCV S/CO ratio가 5이상인 예의 90% 이상에서 혈중 HCV-RNA가 검출된 반면 4-RIA indeterminate 또는 음성이거나 anti-HCV S/CO ratio가 5이하인 경우에는 19예 중 2예에서만 검출되었다.
결론: 우리나라 성인에서 2세대 anti-HCV 발현율을 1.77%이었다. 혈청 ALT치가 반복검사하여 정상이고 anti-HCV 양성인 예들의 약 40%에서 혈중 HCV-RNA가 검출되었으며, anti-HCV S/CO ratio가 5이상으로 높거나 4-RIBA가 양성일 때에는 혈중 HCV-RNA가 검출될 가능성이 매우 높은 것으로 사료된다.
Objectives : The prevalence of anti-HCV among blood donors is about 0.3 to 1.5% when assessed by first or second generation EIA. The significance of anti-HCV in a low risk population such as blood donors is unclear. To estimate the prevalence and significance of anti-HCV (second generation EIA) in Korean adults, we tested 4-RIBA and HCV-RNA RT-PCR in the sera of anti-HCV positive cases with normal serum ALT level.
Methods : Serum samples from 5707 adults who visited Health Counseling Center were tested for ALT, HBsAg and anti-HCV by second generation EIA (UBI). Anti-HCV positive cases with normal serum ALT participated in a brief questionnaire and serum samples of them were tested for HCV by a 4-RIBA and HCV-RNA by RT-PCR.
Results: The overall prevalence of anti-HCV among Korean adults is 1.77% and among adults with normal serum ALT is 1.45%. History of prior blood transfusion was more frequent in anti-HCV positive cases than anti-HCV negative cases.
In 28 anti-HCV positive cases with normal serum ALT level, 4-RIBA was reactive in 35.7% and HCV-RNA was detected in 39.3%. HCV-RNA was detected in 9 of 10 4-RIBA reactive cases but in only 2 of 18 indeterminate or nonreactive cases. All 9 cases with sample/cut-off ratio of greater than 5 in anti-HCV EIA were 4-RIBA reactive and contained HCV-RNA, but only 2 of 19 cases with sample/cut off ratio less than 5 contained HCV-RNA.
Conclusion: The overall prevalence of anti-HCV among Korean adults is 1.77%. About 40% of anti-HCV positive cases with normal SALT level contained serum HCV-RNA by RT-PCR. Reactive results of 4-RIBA and high sample/cut-off ratio in anti-HCV EIA correlated well with detection of HCV-RNA by RT-PCR.
