The positivity and S/N ratio of IgM anti-HBc were assessed in 283 patients with HBV infection by radioimmunoassay. Among the 59 patients with acute viral hepatitis, 45(76.3 percent) had detectable IgM anti-HBc, and S/N ratio was above 6 in all 16 patients who had seroconversion of HBsAg within 6 months. Among the 171 patients with chronic liver disease and 53 healthy carriers, 53(23.7 percent) had detectable IgM anti-HBc, but only 7(3.1 percent) had S/N ratio above 6. In 88 patients with chronic active hepatitis, 42.0 percent of patients had IgM anti-HBc, and there was no difference in positivity or S/N ratio of IgM anti-HBc between HbeAg positive and negative groups. It is suggested that S/N ratio above 6 rather than positivity of IgM anti-HBc is more useful for differentiating recent HBV infection from remote one.-
