Management of postoperative recurrence. Present, future

We aimed to determine the risk factors of recurrent gastric cancer and to evaluate the result of therapy. The main treatment for recurrence was systemic or local chemotherapy. Surgery was performed in selected patients but the incidence of curative resection was extremely low. The first order in the management of the recurrence is to perform curative resection in initial operation and IPHC or CHPP as needed. Secondly careful and regular follow-up is required far detection at an early stage through further improvements of diagnostic techniques, methods and further studies in tumor markers and molecular biologic tests, especially in the high risk group, also aggressive curative re-operation is recommended with combined therapy when indicated. Finally development of new diagnostic methods, chemo-therapeutic regimens with biochemical modulation and curative operative techniques should be pursued.
