Purpose: The purpose of this study is to investigate how the initial stability and the bone-cup con- tact are affected by the varied amount of underreaming. Materials and Method : Three different sized hemispherical Trilogy acetabular cups(Zimmer, Warsaw, Indiana, USA); 48mm, 54mm, 60mm cups were tightly fitted into the distal part of sixty Korean bovine femurs that were underreamed by 1 mm, 2mm, 3mm, and 4mm respectively. The gaps between the acetabular cup and surrounding bone were measured by radiography at the three different portion, polar 1/3, middle 1/3, peripheral 1/3. The micromotions of the acetabular cups were measured by servohydrulic tension-compression machine (Instron-1331, Canton, Massachusetts, USA) under the axial load of 300Kp in each other. Results: The gaps between the acetabular cup and surrounding bone were increased by increasing the underreaming size in all three portions. In the axial load test, the micromotions were decreased by increasing the underreaming size in three different sized cups. Conclusion: Considering both the cup-bone contact and initial stability, 2mm underreaming is rec- ommended for the adequate bone-cup contact without sacrifice of initial stability despite the differ- ence of cup size.
