Gianturco형 스텐트가 가토 기관에 미치는 영향에 관한 실험적 연구

초록

An expanda ble stainless steel stent was formulated for use in treatem ent of tracheobronchia l stenosis, trach eomalacia a nd air way collapse following tracheal reconstruc tio n. The stents were placed throughout an e n. dobronchia l tube into the trachea of 20 h ealthy rabbits‘ Two different of sizes (lOmm , A type & 20mm, B type in diam eter a nd length) and two kinds of sh a pes (standard. A & B and modified. A‘ &B’ with barbs) were used. Group 1 were used with A and A' type stents and group 11 were used with B & B‘ types The a nima ls were observed under fluoroscopy and findings were la te r confirmed at autopsy. With the exception 。 f2 cases where the stents migra ted , 1 case in A subgroup and A' and a n inflammatory reaction was noted in eac h a nima l. Group 11 rabbits we re almost died within 2-4 weeks after stent placem en t. In th e compa rison of the pathohistologic findings between group 1 a nd 11 , the tissue n ecrosis a t the sites of stent placement in trachea is more severe & ex tensive in group 11 than that of group 1, and also reactive fibroblastic reaction is more severe in group 11. So the thickness of the tracheal wall in group 11 is two or three times thicker than that of group 1 whic h seems to be a result of reactive fibrosis. So‘ A&A‘ type stent is safer th a n B & B’ type stent in norma l rabbit trach ea It is a dvisable to use stents in diam eter under 2 times to trach eal width a nd the complication of stent placem e n t in trachea may be reduced for appropriate reconstruction of stenotic lesion
