D-xylose에 依한 腸吸收 機能의 臨床的 硏究

The D-xylose excretion test has been used in the study on the absorption of carbohydrates by the small intestine. In the present study the author dealed with the state of absorption of carbohydrates in conditions, such as general digestive diseases, diseases of the liver and normal subjects, using the D-xylose tolerance test with a five gram dose. The results of urinary xylose concentions in various conditions following in ingestion of 5 grams of D-xylose have been disclosed as below; The mean excretion value for the control group was 1.76±0.20 gram per 5 hours with a range of 1.40 to 2.21 gm. The proportion of D-xylose excretion was decreased with increasing age. A decrease in the D-xylose absorption was significantly noted by statistics in uncomplicated liver cirrhosis, complicated liver cirrhosis with ascites, typhoid fever, tuberculous peritionitis and gastric cancer, comprising of their excretions with 1.03±0.53, 1.29±0.19, 1.30±0.21 and 1.18±0.47 gm, respectively. There was not statistically significant to be a decrease in the D-xylose excretion in the digestive diseases with anorexia compared with them without anorexia. It was statistically high significance to be a decrease in the D-xylose excretion noted in digestive diseases with constipation or diarrhea over a week's duration, and more with anorexia, Urinary xylose excretions were increased with improved symptoms and states of diseases, Ninteen patients with the reversed A/G ratio among the live diseases have the mean urinary xylose excretion of 1.18±0.30 gm per 5 hours and mean serum albumin of 2.81±0.82 gm/dl with a range of 1.2 to 4.0 gm/dl. Thirteen (68.4%) out of 19 patients with the reversed A/G ratio in liver cirrhosis had urinary xylose excretion lavels lower than the lowest recorded in control series, and serum albumin levels of eleven (57.8%)out of them were below 3 gm/100㎖. In 45 patients with non-reversed A/G ratio out of 65 patients with liver disease, urinary xylose mean value was 1.43±0.36 gm per 5 hours with a range of 0.80 to 2.10 gm, and serum albumin, 4.35±0.45 gm/100㎖ with a range of 3.4 to 5.6 gm/100㎖. Seven (19.4%) out of 36 patiesnt with liver cirrhosis had the normal values in urinary xylose excretion and serum albumin levels over 3 gm/100㎖.
