PARKINSONISM AND RELATED DISORDERS 134 (2025) 107554 ASSOCIATION BETWEEN BASELINE IMPULSE CONTROL DISORDER AND FUTURE LEVODOPA INDUCED DYSKINESIA IN PARKINSON’S DISEASE: THE PPMI COHORT

초록

Background: Levodopa induced dyskinesia (LID) is a common motor complication in Parkinson’s disease (PD) along disease progression. Multiple risk factors are known to be associated with the risk of LID, but relationship between impulse control disorder (ICD) and LID is not well elucidated. Therefore, we investigated whether baseline ICD was associated with future LID risk in PD. Methods: This study included patients without LID at baseline from Parkinson’s Progression Markers Initiative (PPMI) database. Four hundred sixty-four patients with PD without LID at baseline were investigated. Presence of ICD was defined as ≥1 on questionnaire for impulsive-compulsive disorders in Parkinson’s disease (QUIP-CS). Kaplan-Meier curve analysis, Cox-regression analysis was used to impact of ICD on LID development. Results: Development of LID was more frequent in PD patients with ICD, and this association was significant considering nigrostriatal dopamine depletion. Moreover, survival analysis revealed that ICD was associated with future LID risk, after considering the effect of putaminal dopaminergic depletion. In addition, baseline QUIP score was associated with LID risk quantitatively. Conclusions: These results imply that baseline impulse control disorder is associated with future LID risk in patients with PD, which might be independent of nigrostriatal dopaminergic degeneration.
