New TSAD-based systems expanding the scope of distalization: Clinical insights and applications

The decision between extraction and nonextraction treatment remains a fundamental challenge in orthodontics. Molar distalization as a nonextraction treatment strategy has gained renewed interest owing to advances in skeletal anchorage systems. This article introduces cases of severe maxillary crowding resolved by distalization with the modified C-palatal plate (MCPP) and bimaxillary protrusion treated using mini-implants and ramal plates. It highlights how treatment strategies may vary depending on the type of temporary skeletal anchorage devices. Also, four key clinical questions regarding nonextraction treatment are addressed: Is it feasible to distalize the maxillary first molar when the second and third molars have not yet erupted? What is the amount of molar distalization that can be achieved using an MCPP and a ramal plate? Does molar distalization with an MCPP reduce airway space and increase the risk of airway problems? Finally, is first molar distalization possible without root resorption in cases of severe sinus pneumatization? Therefore, a comprehensive understanding of the biomechanical principles and clinical applications of MCPPs and ramal plates is essential to achieving a successful nonextraction treatment outcome. The present article illustrates these concepts through representative clinical cases, emphasizing their practical application in contemporary orthodontic treatment.
