Comparison of Effects of Enflurane and Propofol on Core Temperature

Background: Intraoperative hypothermia is common and causes potentially serious complications. Core temperatures in patients who received propofol were consistently lower than those in patients who received inhaled sevoflurane. We compared the thermoregulatory effect on core temperature between enflurane and propofol. Methods: Twenty-eight patients undergoing transabdominal hysterectomy were randomly allocated 2 groups. patient received either thiopental (n = 15, enflurane group) or propofol (n = 13, propofol group) as an induction agent and anesthesia were maintained with either enflurane-N_2O-O_2 or propofol-N_2O-O_2. We measured core temperature, forearm temperature, fingertip temperature at preinduction period, postinduction 10, 20, 30, 40,50, 60, 90 minutes. Fingertip blood flow was estimated using forearm minus fingertip skin temperature gradients, with a gradient of 0℃ identifying onset of vasoconstiction. Results: Core temperatures in patients who received propofol and enflurane decreases during operation and there was no difference between 2 groups. In both groups, forearm minus fingertip skin temperature gradients was positive in preinduction period, but it was negative in postinduction 10, 20, 30, 40, 50, 60, 90 minutes and there was no difference between 2 groups. Conclusions: Reductions in core temperature and vasoconstriction threshold are similar during propofol and enfluane anesthesia.
