全身麻醉中 動脈血 酸素分壓이 2,3-Diphosphoglycerate値에 미치는 影響

During general anesthesia the oxygen carrying capacity and releasing ability of red brood cells to tissue are very important. One of the important factors that cause shift of oxygen dissociation curve of hemoglobin is 2,3-diphosphoglycerate. It decreases the affinity of oxygen to hemoglobin thus improving tissue oxygenation. The purpose of this study is to investigate the relationship between 2,3-diphosphoglycerate levels and arterial oxygen tension during halothane-nitrous oxide-oxygen anesthesia. Sixty-five patients in both sexes were investigated. They were devided into three groups. In group A the flow rate of nitrous oxide and oxygen was 3l/Min. and 1l/Min. in group B, 2l/Min. and 2l/Min., and in group C it was 1l/Min. and 4l/Min. respectively. During anesthesia controlled ventilation was performed by anesthetic ventilator with the aid of pancuronium bromide. Respiratory rate was set around 8 to 10 times per minute. And with the aid of Engstro¨m Eliza Duo CO₂ and O₂ analyzer, end tidal concentration of CO₂ was checked and it was set around 5.0 to 5.5 per cents by the control of respiratory tidal volume. Arterial blood gas analysis was performed one hour after induction of anesthesia. The pH and PCO₂ were within normal range and the PaO₂ of the group A.B.C. were 109.2±17.6mmHg, 247.0±37.1mmHg, 403.8±40.8mmHg respectively. The blood samples were collected just before, one hour after induction, and at the conclusion of anesthesia. The levels of 2,3-DPG were measured by ultraviolet enzymatic method of Sigma Chemical Company. All the three groups showed no change in 2,3-DPG levels.
