This case report presents a nonextraction treatment protocol for enhanced total maxillary arch distalization using a modified C-palatal plate (MCPP) in combination with clear aligner therapy. A 15-year-old male with a Class II malocclusion and excessive overjet was treated using skeletal anchorage provided by an MCPP and a palatal retraction arch. This protocol involved distalization of posterior teeth followed by anterior retraction, enabling controlled, bodily tooth movement with minimal reliance on patient compliance. Treatment outcomes included effective maxillary distalization and elimination of undesirable proclination, resulting in a stable occlusion after 22 months of treatment. This treatment strategy demonstrates the biomechanics of combining MCPP and clear aligners to improve treatment efficiency. Total arch distalization using MCPP and clear aligner therapy offers a reliable alternative to traditional protocols for correcting Class II malocclusion compared with the sequential distalization approach. © 2026 American Association of Orthodontists.
