놀이를 통한 천식 환자 교육 프로그램 개발

초록

목적: 만성 질환으로 소아 천식에 대한 이해와 약물치료의 많은 발전에도 불구하고 이에 관한 천식의 이환율과 사망률 모두 증가하고 있다. 따라서 최근 천식의 관리와 치료의 한 방법으로 천식 교육 프로그램의 개발과 실행이 이루어지고 있다. 이에 천식 소아들의 호기심을 유발할 수 있고 일상 생활에 쉽게 접목시킬 수 있는 천식 교육 프로그램으로 '맑은 숨 천식놀이'를 개발하여 효과적인 천식 관리를 시도하고자 하였다. 방법: 고려대학교 안암병원 소아과 알레르기 클리닉에서 추적 관찰 중인 천식 소아 중 2000년 여름 천식 캠프에 참여한 20명을 대상으로 시행하였다. 놀이는 4명의 경기자가 한 놀이판을 구성하였고, 주사위의 수만큼 자신의 말을 움직여 깃발이 그려져 있는 골인 지점에 먼저 도달하는 경기자가 이긴다. 경기 과정 중 질문카드의 문제를 푸는 과정에서 천식에 대한 일반적인 지식, 천식의 치료와 이에 사용하는 약물, 천식의 예방법, 천식 발작시 대처하는 방법을 습득하도록 하였다. 결과: '맑은 숨 천식놀이'의 목적은 천식에 관한 기초적인 정보를 제공하여 천식의 병태생리에 대한 이해를 돕는다. 천식의 장기적인 관리방법을 습득하도록 한다. 천식 발작을 예방하는 행동을 습득하도록 한다. 천식 발작이 발생하였을 때 대처할 수 있는 방법을 이해하고 실천하는데 자신감을 가질 수 있도록 설정하였으며 놀이를 시행하면서 자연스럽게 내용을 습득할 수 있도록 한다. 결론: 이 연구를 통하여 천식 교육 프로그램을 바탕으로 한 천식 놀이 프로그램의 모델을 제시하는 바이며, 이를 바탕으로 천식 소아들을 위한 놀이를 활성화시킬 계획이며, 향후 천식 놀이 프로그램의 교육효과에 대한 평가가 필요하리라 생각된다. #초록# Purpose: The increased prevalence of childhood asthma in Korea from 3.4% in 1964 to 15% in 1995 might be very important for morbidity and mortality in children. Recognition of this led to consider the necessity of new management strategies emphasizing patient education. We developed the educational program including asthma-specific play game called 'Malgunsum asthma game' to improve the child's interest and evaluated its educational effect. Methods: Twenty asthmatic children between the age seven to fifteen were attended Malgunsum Asthma Camp. The Children were randomly assigned to 5 groups, and each player had to move the horse by answering the questions on the card. The first one who reaches the goal wins the game. Results: All the children well followed the play rule and were interested to the game. Conclusion: This educational game model will achieve better control of asthma by leading the patients to understand the nature of asthma, how to identify and control the aggravating factors, how to recognize symptoms and appropriate steps for asthma, and the importance of daily maintenance therapy with anti-inflammatory medications.
