Analysis of Cough Characteristics in Chronic Respiratory Diseases

Rationale: Cough is a common and distressing symptom in patients with chronic respiratorydiseases, including chronic obstructive pulmonary disease (COPD), asthma, bronchiectasis, andinterstitial lung disease. Cough also impacts their quality of life. The COugh Assessment Test(COAT) is a short patient-completed questionnaire developed to measure the impact of cough onchronic cough patients in a clinical practice. This study investigated COAT's usefulness inevaluating cough in patients with chronic respiratory disease. Methods: A multi-centered cross-sectional study of patients with coughing was conducted in those with respiratory diseases,including asthma, COPD, bronchiectasis, or idiopathic pulmonary fibrosis (IPF). We measuredCOAT, Cough Symptoms Score (CSS), Leister Cough Questionnaire (LCQ), and CoughHypersensitivity Questionnaire (CHQ). The scores of the questionnaires were compared betweendisease groups. Results: We recruited 211 patients. The total score of COAT was highest inpatients with asthma (8.9±4.9) compared to bronchiectasis (5.1±4.2) and COPD (5.3±3.8, p-value<0.05 in Turkey’s host hoc test). It was similar between asthma and IPF (7.9±4.9). In contrast, thetotal LCQ scores were higher in COPD (15.54±3.73) and bronchiectasis (15.11±4.30) compared toasthma (12.84±4.25). IPF’s LCQ score (14.09±4.29) was between them. Cough symptom scores inthe daytime were higher in asthma (2.7±1.1) and IPF (2.6±1.1) compared to COPD (2.1±1.2) andbronchiectasis (2.1±1.2). Similarly, cough symptom scores at nighttime were higher in asthma(1.6±1.2) and IPF (1.7±1.2) compared to COPD (2.1±1.2) and bronchiectasis (2.1±1.2).Conclusion: COAT could help measure cough in chronic respiratory disease. Cough-related qualityof life and burden of cough differed between COPD, asthma, bronchiectasis, and IPF.
