Impact of Statin Therapy on Long-term Clinical Outcomes in Coronary Artery Spasm Patients without Dyslipidemia and Coronary Artery Stenosis

Background: Many of studies presented that Statin has a variety of pleiotropic effect for reducing incidence of cardiovascular events among patients (pts) with coronary artery disease (CAD). However, there were limited data regarding the impact of statin therapy on long-term clinical outcomes in coronary artery spasm (CAS) pts without dyslipidemia and coronary artery stenosis. Methods: A total of 3,729 patients with near-normal coronary artery (< 30% diameter stenosis, DS) who underwent the acetylcholine (ACH) or ergonovine (ERG) provocation test between January 2004 and December 2021 were enrolled. Among these pts, 1,371 who were positive provocation test and had no dyslipidemia were divided into two groups: one treated with a statin (the statin group; n=354) and the other treated without a statin (the control group; n=1,017). We compared 10-year major clinical outcomes between the two groups. Propensity score matching (PSM) was used to adjust for baseline confounders. Results: After PSM, baseline characteristics were balanced between the statin group and the non-statin group. After cox proportional hazards regression, there were no significant difference in the incidence of individual and composite clinical outcomes up to 10 years (Table). Conclusion: In our study, the statin therapy was not associated with improving major clinical outcomes in patients with CAS without dyslipidemia and coronary artery stenosis up to 10 years.
