임상에서 제거된 Core-Vent 임프란트주위조직:투과전자현미경 관찰
초록
Electron microscopy is widely used in the field of dental research. However, technical difficulties are frequently meet in the processing of hard mineralized tissue for transmission electron microscopy. The purpose of this study is an evaluation of the ultrastructural aspects of peri-implant tissues and the evidence of failure at bone-implant interface. 4 failed basket type dental implants were retrieved from a 66 year-old Korean male patient, causes of mobility, pain and discomfort on 5 months after implants prosthetic functioning. Peri-implant tissues and implants were immediately immersed in 2.5% glutaraldehyde and post fixation was done by osmium tetroxide. After dehydration and embedding in Epon, semi-thin section stained by toluidine blue and thin section was done by lead citrate and uranyl acetate for transmission electron microscopic observation without decalcification procedure. The obtained results as follows: 1. Failed implants were encapsulated by connective tissue with inactive or degradative formed mononuclear cells. 2. Transmission electron micrograph demonstraing layers of destructive collagen fibers, demineralizing scattered bony crystals and denudation of the osteocyte by demineralization. 3. Transmission electron micrograph demonstraing, in areas with a close to interface, layers of amorphous substance with collagen fibers and mineralizing bony crystals. These findings, lead to us suggest that initial bone healing process and bone remodeling sequence could be a main contributing factor to sucess of implantation.
