Weber B형 외과골절과 동반된 전방경골-비골인대 견열골절

초록

Purpose: To know the prevalence of the anterior tibiofibular ligament avulsion fracture (Wagstaffe's fracture) combined with the Weber type B lateral malleolar fracture and the relationship with the distal tibiofibular joint diastasis and the proper treatment method. Materials and Methods: We evaluated 93 cases of the ankle fractures treated with operative methods. The radiographs were reviewed and the clinical and radiological results after operation were evaluated. Results: There were 51 cases of Weber type B lateral malleolar fractures and the combined Wagstaffe's fractures were 13 cases (25.5%). The avulsed fragments by the anterior tibiofibular ligament were best observed on the internal oblique views. The distal tibiofibular joint diastases were diagnosed in 20 cases (39.2%) of the 51 cases of Weber type B fractures. In the 13 cases of Wagstaffe's fractures, 11 cases (84.6%) were combined with the ankle diastases. The clinical results were excellent in 9 cases, good in 2 cases and fair in 2 cases and the radiological results were good in 12 cases. Conclusion: The anterior tibiofibular ligament avulsion fracture is one of good diagnostic clue of the ankle diastasis in Weber type B lateral malleolar fracture. We think that the accurate reduction and fixation of the avulsion fragment is important to prevent the chronic ankle pain.
