成熟家兎의 膝蓋骨에 骨씨멘트 接着後 膝蓋骨에 미치는 組織學的 變化에 關한 實驗的 硏究

Recently in the operation of the total hip or knee replacement, the plastic cup and metalic stem being fixed by methylmethacrylate is use widely. Fixation of the acetabular cup on the acetabulum in total hip replacement needs destruction of bone up to cancellous bone layer for acetabular reaming which forces delay in surgical process and induces considerable bleeding. However report on fixability by reaming up to subschondral bone or articular cartilagenous layer instead of cancellous bone layer have been difficult to find. The author performed an experiment to understand fixability and change of the bone and the regeneration process following acrylic bone cement fixation applied to different level of bone destruction such as superficial articular cartilagenous layer, subchondral bon layer and cancellous bone layer, repectively, in the Group Ⅰ, Group Ⅱ and Group Ⅲ. For the study, 96 patellae were obtained from 48 adult rabbits which divided into three groups, giving injuries and applying the bone cement to them respectively. Macro-and microscopic examinations were performed at the intervals of one, two, three, four, six, eight, twelve and sixteen weeks after the surgical manipulation. The results obtained are as follows. 1. Good fixation were obtained in both groups of injury to the subchondral bone layer and to the cancellous bone layer by the fixation with the bone cement throughout the different durations for repair of the bones. There showed not enough fixability of the bone cement in the group of injury to the superficial articular cartilagenous layer regardless the different durations of fixation with the bone cement. 2. The group of injury to the superficial articular cartilagenous layer revealed complete regeneration at the six weeks, Both groups of injuries to the subchondral and cancellous bone layers began to repair at the four weeks and completed between the twelve and sixteen weeks. 3. Bone cement applied to the injured bone induced no significant histologic changes in all the experimental groups.
