Combination treatment of transmitting both Monopolar and Bipolar Radiofrequency treatment for skin laxity: Histological Results and Elasticity Analysis

Background: Monopolar radiofrequency (MRF) has been used as a noninvasive skin tightening and rejuvenation technique. Bipolar radiofrequency (BRF) has also been used for noninvasive skin tightening treatment. Objective: We aimed to evaluate the safety and efficacy of transmitting both MRF and BRF in a combination treatment for facial skin laxity by measuring elasticity. Patients/Methods: Twelve patients with skin types III to V participated in this study. They were treated with an MRF and BRF on the face, and all subjects were followed up for eight weeks after the treatment. Digital photography was taken, and elasticity, viscoelasticity, and retraction time were measured on each visit. Skin biopsies were performed before and two months after treatment. Results: The mean viscoelasticity increased from 4.29 to 5.29 after treatment, and the mean retraction time decreased from 286.83 to 225.17 after treatment. The mean Young's elasticity coefficient increased from 4.28 to 4.39 after treatment. H&E stain showed increased collagen and elastic fibers, especially the dermo-epidermal junction. In Masson trichrome stain, the density of collagen fiber was increased. In the Victoria Blue stain, the density of dermal elastic fibers also increased. No adverse effects were reported. Conclusion: Combining MRF and BRF for facial rejuvenation improves skin laxity through collagen remodeling and elastic fiber reorganization. Histopathological findings and elasticity analysis showed objective improvement in skin laxity. MRF and BRF combination treatments for skin tightening are safe, effective, and noninvasive procedures.
