The second report of web-based multicenter anaphylaxis registry in Korea: clinical features of 808 cases and the treatment pattern in the emergency department

Background: We aimed to report the clinical features and treatment patterns of anaphylaxis registered in the multicenter web-based registry in Korea for 2.5 years. Method: Twenty-three departments from 17 hospitals participated in patient registration from November 2016 to June 2020. Anaphylaxis events within 3 months from the visit date with the exact date of occurrence identifiable were included. Results: During 2.5 years, 808 cases of anaphylaxis were registered, with 64% less than 18 years old. In children, foods (81.2%) were the most common cause, followed by drugs (8.5%), whereas in adults, drugs (59.8%) were the most common cause, followed by foods (26.5%), and insect venom (9.3%). The top 5 food triggers in children were hen’s eggs, cow’s milk, walnuts, wheat, and peanuts. In adults, the top 3 food triggers were shrimp, wheat, and crab, and top 3 drug classes were antibiotics, analgesics, and H2 receptor antagonists. The onset time was ≤ 10 minutes in 41.2%. More than half of children and 46.4% of adult patients experienced anaphylaxis at home. Among the 511 cases registered via the emergency department of participating hospitals, epinephrine was administered in 66.4% (67.9% in children and 64.3% in adults). Conclusion: This second report of multicenter anaphylaxis registry provides the extensive clinical profile of 808 anaphylaxis cases including all age groups. Epinephrine was administered to two-thirds of anaphylaxis patients who visited the emergency department.
