Purpose: The aim of this study was to compare the anatomical characteristics of the trabecular meshwork (TM) and Schlemm's canal (SC) between primary angle-closure glaucoma (PACG) and primary open-angle glaucoma (POAG). Methods: This retrospective study was conducted at a single institution using anterior segment optical coherence tomography. Measurements of TM height, TM thickness, SC area, and anterior scleral thickness (AST) were obtained and compared between the two groups. Linear regression analysis was employed to investigate factors associated with TM height and SC area. Results: In patients with PACG, TM height was significantly lower at 420.30 ± 45.89 μm, and SC area was reduced at 3,503.45 ± 1,112.53 μm2 compared to patients with POAG (TM height: 504.26 ± 55.87 μm, SC area: 4,416.76 ± 1,039.03 μm2 ; all p < 0.001). Linear regression analysis revealed that TM height was shorter with increasing age (β = -0.323), in cases of PACG (β = -0.640), shorter axial length (β = 0.500), shallower anterior chamber depth (β = 0.438), and thinner AST (β = 0.252). SC area was smaller with increasing age (β = -0.245), in cases of PACG (β = -0.389), shorter axial length (β = 0.273), and shallower anterior chamber depth (β = 0.359). Multivariate regression analysis showed that the diagnosis of PACG was associated with TM height and SC area in all models. Conclusions: The observed microstructural differences in the TM and SC in PACG suggest a potential association with the pathogenesis of glaucoma.
