When will voiding difficulty occur after diagnosis of Parkinson's disease? Analysis from 120,626 patients with PD of National Health Insurance Claim data

Introduction & Objectives: Parkinson's disease (PD) is one of the neurological diseases while its impact on voiding function is rarelyinvestigated. The aim of this study is to identify the impact of PD on voiding difficulty (VD) using National Health Insurance Claim data. Materials & Methods: From 224,334 National Health Insurance Claim data patients in 2002 to 2021, 120,626 patients' data wereanalyzed after excluding patients with VD medication, Foley catheter insertion, BPH surgery before PD diagnosis, or withquadriplegia/paraplegia. Medication including alpha blocker/5 alpha-reductase inhibitor, Foley catheter insertion, and benign prostatehyperplasia (BPH) surgery were counted after PD diagnosis. Cox regression analysis was performed to identify the risk factor for VD events. Results: The median duration between PD diagnosis and VD medication start, between PD diagnosis and Foley catheter insertion, andbetween PD diagnosis and BPH surgery were 2.8 years (IQR 1.06, 5.55), 3.77 years (IQR 1.62, 6.87), and 4.04 years (IQR 1.93, 7.07),respectively. Within 3 years of PD diagnosis, 66% (21,609/32,501) of patients with PD started VD medication, 64% (17,479/27,500) had aFoley catheter inserted, and 80% (304/381) got BPH surgery. Any comorbidity such as hypertension, diabetes, dyslipidemia, and stroke wasnot the risk factor for VD events. Conclusions: According to National Health Insurance Claim data, more than half of PD patients developed VD within 3 years after PDdiagnosis. The careful surveillance for VD should be performed in PD patients.
