Triflusal versus aspirin in conjunction with clopidogrel in patients undergoing percutaneous coronary intervention with drug-eluting stent

Background: Triflusal (Disgren®) is an antiplatelet agent that has been shown clinical advantages when compared with aspirin in the secondary prevention of vascular events, especially after ischemic stroke. However, it is unclear whether the triflusal is as safe and effective as aspirin for percutaneous coronary intervention (PCI) with drug-eluting stent (DES). Methods: A total 248 patients (pts, Male 151, mean age, 62.5 ± 10.6 years) who received either aspirin 100 mg/day (n=190) or triflusal 300 mg (n=120) for PCI were enrolled. All pts received 300 to 600 mg of clopidogrel before undergoing PCI and followed by 75 mg/day. Unfractionated heparin (50 U/kg) was administered during PCI. Bleeding and vascular access complications and clinical outcomes up to 1 month were analyzed. Results: Baseline characteristics were similar in both groups. The incidence of major bleeding and vascular complications were not different between two groups (Table). Although it was not statistically significant, triflusal group showed numerically lower rate of overall bleeding and vascular complications. Early clinical outcomes such as death, myocardial infarction, revascularization and MACE were similar up to 1 month. Table. In-hospital bleeding and vascular complications. Conclusions: Triflusal may be associated with similar or superior efficacy over aspirin in reducing periprocedural bleeding and vascular complications without increasing adverse clinical outcomes.
