Mental Disorders in Children of Maternal Multiple Sclerosis and Neuromyelitis Optica Spectrum Disorders: National Cohort Study in South Korea

Introduction: Multiple sclerosis (MS) and Neuromyelitis Optica Spectrum Disorder (NMOSD) are chronic inflammatory CNS demyelinating diseases diagnosed early stage of adulthood, especially women of childbearing age. However, the effects of prenatal exposure to these diseases on the health of offspring, especially mental disorders, is not yet well understood. Objectives/Aims: This study aimed to investigate the risk of mental disorders in the offspring of maternal MS and NMOSD. Methods: This nationwide retrospective cohort study used the Korean National Health Insurance Service database, focusing on offspring of maternal MS/NMOSD from 2005-2019. Propensity score matching and statistical models assessed offspring health risks, considering major congenital malformations and mental disorders. Results: Of 3,524,987 infants included in the study, 257 mothers had been diagnosed with MS and 26 with NMOSD before childbirth. No significant risk of major congenital malformations in MS and NMOSD offspring. MS offspring had an increased risk of mental disorders (odd ratio [OR] 1.64; 95% CI: 1.06−2.54), including neurotic and behavioral disorders, compared to controls. However, offspring of maternal NMOSD showed no significant risk of mental disorders ([OR] 1.95; 95% CI: 0.63-6.06). Conclusion: This study showed increased risk of mental disorders in offspring of maternal MS patients, and no significant increase in offspring of maternal NMOSD.
